三黄抗氧化方抑制乳腺癌患者围手术期应激状态的临床研究

摘要:

目的 观察三黄抗氧化方干预对乳腺癌患者围手术期的氧化应激血清学指标及临床症状的影响及其对血管新生的抑制作用。方法 30例乳腺癌患者随机分为实验组与对照组,每组15例。实验组在基础治疗上予以术前1d至术后7d服用中药三黄抗氧化方(每天2袋,每袋100ml);对照组的基础治疗同实验组,口服与实验组相同包装的生理盐水。两组均于术前1d,术后12h及7d测定患者血清中一氧化氮(NO)、超氧化物歧化酶(SOD)的水平;术前1d和术后6h、12h、24h及48h测定患者血清中肿瘤坏死因子(TNF)-α、白介素(IL)-6和血管内皮生长因子(VEGF)水平及血管内皮细胞增殖率,同时评价患者应激反应的症状积分。结果 实验组患者术后第7d血清中NO水平较对照组降低,SOD水平上升;自术后12~24h,实验组患者的血清TNF-a、IL-6及VEGF水平低于对照组,差异有统计学意义(P<0.05);实验组自术后6h表现为显著抑制血管内皮细胞的增殖(P<0.05);术后24h实验组的全身应激症状积分明显改善,与对照组比较,差异有统计学意义(P<0.05)。结论 三黄抗氧化方干预可以改善乳腺癌围手术期患者全身应激状态的临床症状及氧化应激相关的血清学指标,抑制血管内皮细胞增殖与VEGF表达。

Abstract:

To evaluate the effects of traditional Chinese medicine Sanhuang kangyanghua decoction on the oxidative stress related serological markers, clinical symptoms and the inhibition of angiogenesis for breast cancer patients. Methods Thirty breast cancer patients were randomly assigned to experimental and control groups (n=15). 100 ml of saline was orally administrated twice per day for each patient in control group besides the basic treatment, whereas 100 ml of Sanhuang kangyanghua decoction was orally taken for the experimental group. The levels of nitric oxide (NO) and superoxide dismutase (SOD) in serum were measured at 1d before operation and postoperatively at 12h and 7d. The levels of TNF (tumor necrosis factor)-α, IL (interleukin)-6 and VEGF (vascular endothelial growth factor) in the serum were detected at 1d before operation and postoperatively at 6, 12, 24 and 48h. Meanwhile, the sores of the symptoms caused by oxidative stress were recorded. Results Compared to the Control group, the NO level decreased and SOD level increased on 7d postoperatively in experimental group. The levels of TNF-α, IL-6 and VEGF of Experimental group were lower than those of control group within 12 to 24h postoperatively (P<0.05). At the same time, the sores of the symptoms caused by oxidative stress significantly decreased at 24h postoperatively. Conclusion It can reduce the oxidative stress of breast cancer patients during the perioperative period with the traditional Chinese medicine Sanhuang kanguanghua decoction by its antioxidative and anti-angiogenesis effects, beneficial for the recovery and anti-metastasis for such patients.
