Influences of zoledronic acid and surgery on the serum NTx level of patients with giant cell tumor of bone

摘要: 目的 探讨骨巨细胞瘤(GCT)患者血清中Ⅰ型胶原交联氨基末端肽(NTx)水平以及不同治疗对其水平的影响。方法 35例GCT患者随机分为2组,A组接受唑来膦酸及手术治疗,B组仅接受手术治疗。应用ELISA方法检测患者治疗前后的血清NTx水平。应用单因素方差分析影响患者血清NTx水平的因素。结果A组的血清NTx初始水平为(28.50±7.06)nmol BCE/L,使用唑来膦酸72h后下降为(21.38±3.33)nmol BCE/L,手术2周后下降为(20.60±3.28)nmol BCE/L,使用唑来膦酸和手术后的NTx水平与NTx初始水平的差异均有统计学意义(P<0.05)。B组的血清NTx初始水平为(26.05±4.37)nmol BCE/L,手术2周后下降为(24.38±3.56)nmol BCE/L,差异有统计学意义(P<0.05)。A组治疗后的NTx变化率均高于B组(P<0.05)。单因素方差分析结果显示,肿瘤位置对血清NTx水平有显著影响(P<0.05)。结论 唑来膦酸及手术治疗均可降低血清Ntx水平,且唑来膦酸联合手术的效果较好。NTx具有辅助GCT临床诊治的价值。

Abstract: Objective To detect the serum pyridinoline cross-linked N-telopeptides of type Ⅰ collagen (NTx) level of giant cell tumor of bone(GCT) patients, and to investigate the effects of zoledronic acid and surgery on it. Methods Thirty-five GCT patients were enrolled, and randomly divided into group A and group B. Patients in group A were given zoledronic acid (Zometa) and surgery, while patients in group B only received surgery. Blood collection was performed before and after the treatments. Level of serum NTx were tested by ELISA method. Factors affecting the serum NTx level of GCT patients were analyzed by single factor analysis of variance. Results For group A, NTx levels after zoledronic acid were (28.50±7.06) and (21.38±3.33) nmol BCE/L before and after zoledronic acid(P<0.05), and then dropped to (20.60±3.28)nmol BCE/L after surgery(P<0.05). For group B, NTx levels were (26.05±4.37) and (24.38±3.56) nmol BCE/L before and after surgery(P<0.05). The change rate of NTx after treatment in group A was higher than that in group B(P<0.05). According to statistical analysis, the serum NTx level of GCT patients all decreased significantly after receiving zoledronic acid, surgery and zoledronic acid plus surgery (P<0.05). Tumor location was found to be a significant influential factor of serum NTx level (P<0.05). Conclusion The serum NTx level of the GCT patients can be decreased by either zoledronic acid or surgery,showing the potential value in assisting the diagnosis and treatment of GCT.
