胃癌组织中HER-2及Ki-67的表达及相关性

The correlation of HER-2 gene amplification and Ki-67 protein expression in gastric cancer

摘要: 目的 探讨胃癌组织中人类表皮生长因子受体2(HER2)基因的表达及其与Ki67蛋白表达的相关性。方法 采用免疫组化染色及荧光原位杂交(FISH)法检测156例胃癌组织中HER-2基因扩增及Ki-67蛋白的表达情况,分析其与临床病理参数之间的关系。
结果 156例胃癌组织中HER-2基因扩增率为11.5%(18/156);HER-2基因扩增与肿瘤浸润深度及脉管癌栓有关(P<0.05),而与性别、年龄、肿瘤大小、肿瘤类型和淋巴结转移等无关(P>0.05)。胃癌组织中Ki-67蛋白阳性表达率为73.72%(115/156),高于癌旁组织的2.56%(4/156),差异有统计学意义(P<0.05);Ki-67表达与肿瘤组织学类型及肿瘤浸润深度有关。HER-2基因扩增与Ki-67表达呈正相关(r=0.162,P<0.05)。结论 HER-2基因扩增与Ki-67蛋白表达在胃癌的发生发展过程中有重要作用;HER-2基因状态和Ki-67蛋白的联合检测对于评估胃癌患者病情及制定个体化治疗方案有重要意义。

Abstract: Objective To investigate the expression and relationship of HER2 and Ki67 in gastric cancer, and analyze their correlation with clinicopathological parameters. Methods FISH and immunohistochemical staining were used to detect the HER-2 gene amplification and Ki-67 protein expression in 156 gastric cancer tissues. The association between HER-2, Ki-67 expressions and clinicopathological features was analyzed. The interrelationship of HER-2 and Ki-67 expression in gastric cancer tissues was explored.
ResultsThe amplification rate of HER-2 gene was 11.5%(18/156). The amplification rate of HER-2 gene was significantly related with depth of invasion and cancer embolus(P<0.05). The positive expression rate of Ki67 in gastric cancer tissues was 73.72%(115/156), which was higher than 2.56%(4/156) of paracarcinoma tissues with significant differences(P<0.05). The expression of Ki-67 was significantly related with histological type and depth of invasion. HER-2 amplification and Ki-67 expression was positively correlated(r=0.162, P<0.05). Conclusion HER-2 and Ki-67 expression may be involved in the tumor progression of gastric cancer. The combination detection of HER-2 amplification and Ki-67 expression may be used to evaluate the patients'prognosis and develop an individualized therapy.
