Effect of shRNA mediated P2X7R silencing on cell proliferation, apoptosis and signal pathways of PI3K/Akt and Wnt/β-catenin in nasopharyngeal carcinoma

Objective To explore the effect of short hairpin RNA (shRNA) mediated purinergic receptor P2X ligand-gated ion channel 7 (P2X7R) silencing on cell proliferation, apoptosis and signal pathway in nasopharyngeal carcinoma. Methods After optimization of the transfection conditions of shRNA, two shRNA vector fragments (shRNA-1, shRNA-2) of P2X7R gene were transfected into HONE1 cells, respectively. The HONE1 cells transfected with nonsense sequence was assigned as empty transfection group and cells without any treatment was used as control group. Quantitative real-time PCR (QPCR) was used to detect the expression of P2X7R at 24, 48, 72 and 96 h after transfection and the infection vector with high inhibition rate was screened for subsequent experiment. The proliferation inhibition rate was detected at 24, 48, 72 and 96 h after transfection by thiazolyl blue (MTT). The cell apoptosis was detected by flow cytometry at 48 and 96 h after transfection. The mRNA levels of apoptosis related genes were detected by QPCR method. The protein changes of phosphatidylcholine 3-kinase/protein kinase B (PI3K/Akt) pathway and Wnt/β-catenin pathway were detected at 96 h after transfection by Western blotting. Results The level mRNA of P2X7R in transfected group was lower than those in the blank group and the control group (P<0.05). The shRNA-1 carrier with high interference efficiency was selected for the functional study. Compared to empty transfection group and control group, there were increased inhibition rate, apoptosis rate and mRNA levels of pro-apoptosis protein bid and Bax but decreased mRNA levels of anti-apoptosis protein Bcl-2 and Mcl-1 in shRNA-1 transfected group with statistical difference (P<0.05). Compared with the control group, the protein level of PTEN was elevated and protein level of p-Akt in PI3K/Akt pathway were decreased in shRNA-1 transfection group (P<0.05). As for the Wnt/β-catenin pathway, there were decreased expression of p-β-catenin, Cyclin D1 and C-myc in transfection group (P<0.05). Conclusion The inhibition of P2X7R gene expression by shRNA can inhibit the proliferation and induce apoptosis, which may be related to the inhibition of PI3K/Akt and Wnt/β-catenin pathway, and it has a certain value for the prevention and treatment of nasopharyngeal carcinoma.

Expression of tumor infiltrating dendritic cells marked by CD1a and CD83 in breast cancer and its correlation with prognosis

Objective To explore the expression of CD1a and CD83 marked tumor infiltrating dendritic cells(TIDC) in breast tissues and its correlation with prognosis. Methods Eighty-five cases of breast cancer tissues were collected and CD1a, CD83 expressions of breast cancer tissues were detected by immunochemical method. The relationship between their expression and clinicopathological features and prognosis were analyzed. Results There were 77 cases with CD1a positive expression in 85 cases of breast cancer tissues, and most TIDC expressing CD1a were distributed in tumor tissues, and less expressed in tumor stroma and surrounding tissues; there were 44 cases with CD83 positive expression in 85 cases of breast cancer tissues, and TIDC expressing CD83 were observed in surrounding and interstitial tissues of tumors, with scattering distribution. There were 35 cases with CD1a and CD83 double positive expression. CD1a, CD83 double positive expression had no correlation with lymph node metastasis, progestrogen receptor(PR) and neutrophil-lymphocyte ratio(NLR)(P>0.05), but correlated with estrogen receptor(ER), HER-2 and TNM staging(P<0.05). The median disease-free survival(DFS) of patients with both CD1a, CD83 double positive expression and non-double positive expression were not reached. Multivariate analysis indicated that non-double positive expression of CD1a, CD83 was an independent factor affecting prognosis of patients (HR=2.216,95%CI:1.682-3.431;P<0.05). Conclusion The expression of characteristic markers CD83 and CD1a of TIDC in breast cancer tissues are related with prognosis, and may be associated with immune escape mechanism of tumors, so they can be the reference indicators to detect the prognosis of breast cancer.

Expression of lncRNA HOTAIR in non-small cell lung cancer and its clinical significance

Objective To investigate the expression level and clinical significance of long chain non encoded lncRNA HOTA IR in non-small cell lung cancer (NSCLC). Methods Ninety-one cases of NSCLC patients diagnosed by pathology from January 2010 to January 2013 in our hospital were enrolled. Ninety-one cases of surgical resected cancer tissues and 62 cases of paired adjacent tissue samples were collected. HOTAIR levels were detected by quantitative real-time PCR (QPCR) method. The relationship between HOTAIR level in NSCLC tissues and clinical pathological parameters (gender, age, TNM stage, tumor size, differentiation, lymph node metastasis, pathologic type and CEA level) were analyzed. The prognosis of different HOTAIR levels was analyzed according to the follow-up data. Results The relative level of HOTAIR in NSCLC tissues was 6.271±0.884, higher than that of adjacent cancer tissue with statistical significance (P<0.05). The expression of HOTAIR in NSCLC was not related to gender, age, degree of differentiation, pathological type and CEA abnormality, but related with TNM stage, tumor size and lymph node metastasis. The levels of HOTAIR in patients of TNM stage Ⅱ-Ⅲ, tumor size more than 3 cm or with lymph node metastasis were higher than those of corresponding items (P<0.05). The median overall survival(OS) of 91 patients was 36.4 months, unrelated with gender, age, degree of differentiation and serum CEA level, but related with TNM stage, tumor size, lymph node metastasis and HOTAIR expression. The median OS of patients with HOTAIR high-expression was 27.8 months, lower than 36.4 months of patients with low-expression with statistical significance (P<0.05). Multivariate analysis showed that TNM stage, tumor size, lymph node metastasis and HOTAIR expression were independent factors influencing the prognosis of NSCLC patients. Conclusion HOTAIR is highly expressed in NSCLC tissues, and it may be related to TNM stage, tumor size, lymph node metastasis and prognosis. It may paly a role in the occurrence and development of NSCLC.

Objective To examine the expression of Dishevelled-1 (Dvl1), Cyclin D1 and CD44v6 in pediatric osteosarcoma tissues, and investigate the correlations among them and the clinicopathological factors. Methods The expressions of Dvl1, Cyclin D1 and CD44v6 were detected in 47 pediatric osteosarcoma tissues by immonohistochemistry method. The relationship among Dvl1, Cyclin D1, CD44v6 and clinicopathological factors was investigated. Results The high expression rates of Dvl1,Cyclin D1 and CD44v6 were 76.60% (36/47), 72.34% (34/47) and 68.09% (32/47), respectively in pediatric osteosarcomas. The expression of Dvl1 was correlated with the tumor diameter (P=0.020) and Ennecking stage (P=0.032). Similarly, the expression of Cyclin D1 and Cd44v6 were also correlated with the tumor diameter (P=0.007 and P=0.036) and Ennecking stage (P=0.024 and P=0.020) of pediatric osteosarcomas. Moreover, the expression of Dvl1 was positively correlated with Cyclin D1 expression (r=0.332,P=0.023) and CD44v6 expression (r=0.376,P=0.009), respectively. Conclusion Dvl1, Cyclin D1 and CD44v6 were highly expressed in pediatric osteosarcoma. The high expressions of Dvl1, Cyclin D1 and CD44v6 can promote the malignant progression of pediatric osteosarcomas.

Meta analysis of the association between miRNA-196a2 rs116149131 polymorphism and gastric cancer risk

Objective To evaluate the association between the microRNA (miRNA)-196a2 rs116149131 gene polymorphism and susceptibility of gastric cancer. Methods All studies published up to March 2016 on the association between miRNA-196a2 rs116149131 gene polymorphism and susceptibility to gastric cancer were searched from PubMed, EMBASE, Cochrane library, CNKI, VIP, Wan fang and CBD database. The data were screened according to the inclusion and exclusion criteria, and extracted, and the quality of included studies was evaluated.The pooled odds ratios (OR) with 95% confidence intervals (95%CI) were calculated using Stata 12.0 software. Publication bias and sensitivity analysis were also assessed. Results A total of 9 studies involving 3992 cases and 5699 controls were included in the metaanalysis. The present meta-analysis showed no association between the miRNA-196a2 rs116149131 gene polymorphism and gastric cancer risk. In ethnic subgroup analysis, a strong association in heterozygous model (CT vs. TT: OR=1.93, 95%CI:1.35-2.75, P<0.05) was found between rs11614913 and gastric cancer risk among Caucasians but no association in all models was found among Asians. In source of controls subgroup analysis, no association was found between rs11614913 and cancer risk among population-based (PB) and hospital-based (HB).In the subgroup of T<C, remarkably elevated gastric cancer risks were found in dominant model (CT+CC vs. TT:OR=1.40, 95%CI:1.10-1.77, P=0.005), homozygote model (CC vs. TT:OR=1.59, 95%CI:1.22-2.06, P=0.001) and heterozygote model (CT vs. TT:OR=1.33, 95%CI:1.04-1.70, P=0.025). We did not find significant associations in recessive model and allele model. No association was found between rs11614913 and cancer risk in the subgroup of T>C. Conclusion miRNA-196a2 rs116149131 gene polymorphism and gastric cancer has no risk associated,but there are racial differences.

Plasma level of microRNA-510 and efficacy of oxaliplatin-based chemotherapy in advanced gastric cancer

Objective To investigate the relationship between the plasma level of microRNA-510 (miR-510) and efficacy of oxaliplatin-based chemotherapy in advanced gastric cancer. Methods Peripheral blood samples from 112 advanced gastric cancer patients treated with oxaliplatin-based chemotherapy were collected in our hospital. The level of miR-510 was measured by real-time quantitative PCR. The RECIST 1.1 was employed to evaluate the short-term efficacy of oxaliplatin-based chemotherapy in patients after 2 cycles. The patients of CR and PR were assigned as the sensitive group, while those of SD and PD were not sensitive group. The relationship between the sensitivity of chemotherapy and the plasma level of miR-510 were analyzed. The long-term survival of different plasma miR-510 levels were compared according to the follow-up data. In the same period, 94 peripheral blood samples from the hospital’s health examination were selected as control group. Results The relative levels of plasma miR-510 was 3.417±0.574 in patients with gastric cancer, higher than the control group(P<0.05). The expression of miR-510 in the plasma of patients with gastric cancer was related to TNM stage, degree of differentiation, lymph node metastasis and CEA level(P<0.05). Of the 112 patients with advanced gastric cancer patients after 2 cycles of chemotherapy, there were 48 cases of PR, 37 cases of SD and 27 cases of PD. The plasma level of miR-510 in 48 patients with chemotherapy sensitivity was 2.362±0.534, less than 4.208±0.602 of the patients who were not sensitive to chemotherapy (P<0.05). In the whole group, the median progression-free survival (PFS) and overall survival (OS) were 7.6 months and 11.5 months, respectively. The patients were divided into miR-510 high-level group and low-level group according to the median value of 3.053. The median PFS and OS were 5.3 and 10.2 months of miR-510 high-level group, less than 8.4 and 12.4 months of miR-510 low-level group (P<0.05). Conclusion The plasma miR-510 levels were elevated in the patients with gastric cancer, and associated with the sensitivity of oxaliplatin in advanced gastric cancer. The patients with higher miR-510 level had a poor prognosis, which has a value for predicting the effect and prognosis of patients with advanced gastric cancer treated with oxaliplatin.

Clinical efficacy of synchronous intensity modulated radiation therapy combined with weekly dose of paclitaxel and cisplatin for locally advanced esophageal cancer

Objective To investigate the efficacy and safety of synchronous intensity modulated radiation therapy (IMRT) combined with weekly dose of paclitaxel and cisplatin for locally advanced esophageal cancer. Methods From January 2010 to April 2015, 36 patients with previously untreated locally advanced esophageal cancer were assigned to receive weekly paclitaxel (50 mg/m2) and cisplatin (25 mg/m2) plus concurrent IMRT. The total radiation dose was 60 Gy in 30 fractions with 5 fractions a week. Response to radiochemotherapy was assessed by RECIST criteria 1.1 and toxicity was evaluated according to National Cancer Institute Common Toxicity Criteria 4.0. The clinical follow-up data from radiochemotherapy were investigated. Results All patients were available for evaluation. Seventeen got complete response, 13 cases had partial response, 4 cases had stable disease and 2 cases had progressive disease. The response rate was 83.3% and the disease control rate was 94.4%. Of the 36 patients, the median progression-free survival was 13.0 months and median overall survival was 25.0 months. The 1-, 2-, 3-year survival rates were 77.8%, 63.9% and 33.3%. Grade 3 or higher major toxicity consisted of leukopenia (25.0%),febrile neutropenia (11.1%), anorexia (27.8%) and esophagitis (25.0%). Conclusion The IMRT combined with weekly dose of paclitaxel and cisplatin is effective and tolerated in the treatment of locally advanced esophageal cancer.

Analysis on dosimetry of domestic medical linear accelerator for early stage cancer after breast-conserving surgery

Objective To evaluate the characteristics of dosimetry and clinical efficiencies of domestic medical linear accelerator, and provide dosimetry basis for the clinical application of domestic linac. Methods Eleven patients were selected after breast conserving surgery who need to adjuvant radiotherapy, including 5 right breast cancer patients and 6 left breast cancer patients. The prescription dose was 50Gy/25f. Each patient performed dynamic intensity modulated radio-therapy (dIMRT) and three-dimensional conformal radiotherapy(3D-CRT) by TIGRT TPS. The dose volume histograms (DVHs) were used to analyze the characteristics of dosimetry on target volume and the organs at risk (OARs), and the conformity index (CI) and homogeneity index (HI) were calculated. The monitor unit (MU) and performing time was recorded. Results There was similar coverage of target dose between dIMRT and 3D-CRT. D95 and V100 of dIMRT and 3D-CRT plan were (5009.73±12.70)cGy, (95.41±0.54)% and (5005.47±7.30)cGy, (95.18±0.28)%. D2, CI and HI of dIMRT and 3D-CRT plan were (5551.66±89.21)cGy, 0.83±0.01,1.10±0.02 and (5819.20±111.19)cGy, 0.52±0.05, 1.14±0.02,with significant differences(P<0.05). V5, V10 and V20 of patients with ipsilateral lung of dIMRT was (65.32±5.88)%, (38.99±14.42)% and (17.76±0.43)%, which was significantly higher than that of 3D-CRT plan(P<0.05). V30of patients with ipsilateral lung of dIMRT plan was(5.59±1.52)%,which was significantly lower than that of 3D-CRT plan,with significant differences(P<0.05). Dmeanof left breast cancer patients heart in dIMRT plan was(814.59±154.82)cGy, which was higher than that in 3D-CRT plan, with significant differences(P<0.05); Dmaxof left breast cancer patient’s heart in dIMRT plan was(4067.93±441.39)cGy, which was lower than that in 3D-CRT plan, with significant differences(P<0.05). Dmax and Dmeanof right breast cancer patient’s heart in 3D-CRT plan were(56.34±15.56)cGy and(393.00±94.95)cGy,which were lower than those in dIMRT plan, with significant differences(P<0.05). MU and treatment delivery time of dIMRT plan was 593.17±39.27 and (124.57±8.25)s,while that in 3D-CRT plan was 639.50±49.81 and (134.30±10.46)s. Conclusion Domestic linac can be perform well in rediotherapy for early stage cancer after breastconserving surgery, and dIMRT with good at CI and HI, 3D-CRT has lower sparing at OARs.

Impacts of rectum filling status on dosimetric parameters of target volume and organ at risk in intensity-modulated radiotherapy for prostate cancer

Objective To investigate the impact of rectum filling status on the dosimetric parameters of target volume and organ at risk (OAR) in intensity-modulated radiotherapy (IMRT) for patients with prostate cancer. Methods Fifteen patients with prostate cancer were enrolled in this study. Target and OAR were contoured by the same doctor both when the rectum was full and empty. And the IMRT plan was designed by the same medical physicist at the same radiotherapy dose. The impact of rectum filling status in dose of target and OAR during radiotherapy was investigated. Results If the rectum filling status was consistent in orientation and radiation, the rectum fill status was not associated with the dosimetric parameter of target volume and bladder, femoral heads (P>0.05). If the rectum filling status was not consistent in orientation and actual radiation, the dosimetric dose, uniform index and conformal index of target were different(P<0.05). The impacts on bladder and femoral heads were not significant. ConclusionTo guarantee the perfect execution of IMRT in patients with prostate cancer, it is recommended to keep the same rectal filling status during orientation and actual radiation.

Correlation between distress and quality of life in patients with breast cancer after chemotherapy

Objective To explore the relationship among the different degrees of distress and quality of life scores in breast cancer patients after chemotherapy. Methods From March 2015 to March 2016, 60 breast cancer patients received 6 cycles of standard chemotherapy in our hospital were evaluated by distress thermometer (DT) and Functional Assessment of Cancer Therapy-Breast (FACT-B). Depending on whether the DT≥4, patients were divided into high and low groups. The scores of quality of life were compared between both groups. The correlation of distress and quality of life were analyzed. Results Sixty patients completed the questionnaire. The average FACT-B score was 124.3±11.2 in DT low group, while 89.5±15.6 in DT high group, and the difference between the two groups was significant (t=10.06, P<0.05). There was a negative correlation between the DT score and FACT-B score significantly (r=-0.949,P<0.05). Conclusion Patients with breast cancer present different degree of psychological distress after chemotherapy, and the distress scores is closely related to quality of life.

Clinical Observation of the effect of vertebral radiotherapy in the treatment of spinal metastasis

Objective To investigate the clinical efficacy and safety of vertebral radiotherapy in the treatment of spinal metastasis. Methods The data of 116 spinal metastatic tumors patients from September 2012 to February 2016 were retrospectively reviewed. All the patients were equally divided into the observation group and the control group with 58 patients in each group according to treatment. Patients in control group were treated with percutaneous vertebral angioplasty,and those in observation group were given percutaneous vertebral angioplasty combined with radiotherapy (3 Gy/f, 1/d, 5/week, 4 weeks). Vertebral body height recovery rate and Cobb angle recovery rate were observed 1 day and 4 weeks after operation. Complications were recorded as well. Results All the patients accomplished treatment in plan. Pain relief rate in observation group was 93.1%, higher than 79.3% of control group (P<0.05). The vertebral body height recovery rate and Cobb angle recovery rate in observation group were (45.36±10.33)% and (45.96±10.45)%, higher than (42.89±10.41)% and (41.99±8.22)% of control group (P<0.05), and the two indexes were significantly higher 4 weeks after operation than those of 1 day after operation (P<0.05). The main complications of the two groups were bone cement leakage and discomfort of lower limbs, and the side effect of radiotherapy in observation group was mild. Conclusion Vertebral radiotherapy combined with percutaneous vertebral angioplasty in the treatment of spinal metastasis can effectively relieve pain, promote the correction of vertebral deformity with high safety, which is worth of clinical recommendation.

Adenomatoid tumors of the uterus: a clinicopathological analysis of 110 cases

Objective To investigate the clinicopathological features of uterine adnomatoid tumors in order to improve the diagnostic level of this disease. Methods The clinical and pathological data of 110 adenomatoid tumors of uterus in Changzhou Second People’s Hospital Affiliated to Nanjing Medical University from January 2008 to December 2015 were collected to analyze the clinicopathological characteristics. Results Uterine adnomatoid tumors accounted for 1.55% in the contemporaneous hysterectomy and uterus-preserving tumor excision specimen. The clinical manifestations were not specific. A total of 109 cases located in the myometrium, one case located in subserous. Microscopically, the tumors were arranged in glandlike, microcystic, trabecular, solid patterns with flat, cuboidal, epithelialoid tumor cells. Immunohistochemical staining showed tumor cells were positive for CK, Vim, HBME-1, D2-40, Calretinin, WT-1, but negative for CD34, EMA. Conclusion Uterine adenomatoid tumor is not rare and easy to be misdiagnosed in clinical and pathological examination. Immunohistochemistry plays an important role in diagnosis
and differential diagnosis of this disease.

Objective To explore the efficacy and side effects of 5-filuorouracil combined hypofractionated image-guided three dimensional conformal intensity modulated radiation therapy (3D-IMRT) for locally advanced pancreatic carcinoma. MethodsFrom June 2011 to December 2014, a total of 37 patients with locally advanced pancreatic carcinoma proved by histopathology were enrolled in this study. Patients were treated by image-guided 3D-IMRT (radiation dose was 48-49.6 Gy in 16 fractions) and 5-filuorouracil(a dose of 350 mg/m2,d1-d5,weekly). The treatment efficacy and side effects was observed and all patients were followed-up. Results All patients completed 3D-IMRT. There was 5 patients got CR, 14 got PR, 14 got SD and 4 got SD. The response rate was 51.4%(19/37), and clinical benefit response rate was 56.8%(21/37). During the treatment, grade 3 hematologic toxicities occurred in 7 patients, serious biliary obstruction occurred in 1 patient, gastroduodenal ulcer occurred in 2 patients. The 1- and 2-year distance metastasis free survival were 36.0% and 4.8%. The 1- and 2-year overall survival rates were 51.3% and 11.6%. At the end date of the follow-up, 13 patients got local recurrence, 26 patients had distant metastasis. Conclusion 5-filuorouracil combined hypofractionated image-guided 3D-IMRT is effective and well-tolerated for patients with locally advanced pancreatic carcinoma.

Status and thinking of maintenance therapy in patients with pancreatic cancer

The prognosis of advanced pancreatic cancer is extremely poor. According to clinical trials concerned with first-line systemic therapy, the median progression-free survival (PFS) was 5 months and median overall survival (OS) was less than one year. To those patients benefited from first-line regimen and with good performance status, can we switch the therapeutic modality into a single cytotoxic or target agent. It is so called “maintenance therapy”? As concerned to pancreatic cancer patients received radical resection, five-year survival rate was less than 25% even adjuvant therapy completed. May maintenance therapy continue with adjuvant setting aimed to prolong disease-free survival? We combed these burning problems to be solved practically in order to provide clinical reference.

The main object is to reduce or prevent skeletal-related events, prolong survival and improve quality of life. The main treatment includes anti-tumor therapy, bone-modifying agents, surgery, radiation therapy, analgesic treatment and other supportive treatment. This article makes a summary on therapeutic medicine for bone metastasis of breast cancer.

Endoscopic retrograde cholangiocretography(ERCP) and routine cytology are general approaches for pre-oprative dignosis of suspected pancreatic duct cancer patients. Routine cytology is with high specificity and low sensitivity. The detection of aneuploidy of 3/7/17 and loss of p16 by fluorescence in situ hybridization(FISH) has revolutionized how laboratories diagnose cholangiocarcinoma and pancreatic adenocarcinoma with increasing sensitivity and minimal decreases in clinical specificity. Although using FISH for diagnostic aim has been more than ten years in foreign, it has not applied to clinical diagnosis in China. Our hospital has promoted this technique and published an article in Chinese Journal of Pathology. However, many peers in other hospitals wonder detailed methods and criterion of this technique, so we provide the information to detect pancreatobiliary cancers in FISH.
